If you have been approved entry into the Masters program please complete along with the stats form (see below) in order for your enrolment to be processed. If you have not yet submitted copies of your academic records please do so when submitting this form.

Current students who have previously enrolled in the program. Please note that you do not have to resubmit your academic records or stats form unless otherwise instructed to do so. Students must complete and submit a re-enrolment form each study period.
